The role conducts the initial home care assessment and/or re-visit with skilled nursing care and teaching to individuals and their families in their homes during the hours that you are on call. The Start-of-Care/Re-Visit Nurse/Triage Nurse works with other Nightingale team members and the patients care providers; without providing Case Management for the patient.* This is one week on/one week off on call rotation. Monday to Monday. Then off Monday to Monday. Then on Monday to Monday.* You will cover Monday, Tuesday, Wednesday, Thursday,Friday overnights. Then all day Saturday and all day Sunday.Pay: $700/Week that you are on call plus you are paid for patient visits that are performed*Benefits:** *Free Yourself from the burden of OASIS!!
