Job Description & Requirements Specialty: Home Health Discipline: CNA Start Date: 07/16/2025 Duration: Ongoing Up to 40.00 hours per week Shift: 8 hours, days, on call Employment Type: Per Diem Certified Nursing Assistant - (Per Diem) Position Job Summary: The Certified Nursing Assistant (CNA) provides essential personal care services under the supervision of a Registered Nurse. This role involves supporting patients with daily activities and ensuring their comfort and well-being in a home health setting. Key Responsibilities: Travel to patients’ residences to deliver care. Ensure safe and high-quality delivery of home health services. Educate patients, families, and caregivers about diagnoses and care plans. Implement care plans as directed by the case manager. Maintain accurate and timely documentation aligned with care plans. Assist patients with daily activities such as eating, bathing, and mobility. Coordinate with healthcare professionals to ensure comprehensive care. Use medical equipment and supplies efficiently and responsibly. Perform additional duties as assigned by the supervising Registered Nurse. Adhere to the Nursing Practice Act and maintain standards of competent performance. Qualifications: Certification as a Nursing Assistant (CNA). Ability to travel to patient locations. Strong communication and interpersonal skills. Commitment to patient care and safety.
